Overview of the care pathway
The COVID-19 pathway helps to take pressure of the already stressed healthcare systems by supporting patients before they contact caregivers and by following up on patients after a visit to a GP or hospital.
The pathway can be started on a care organisation’s website and provides the first advice to patients based on a screening of the patient’s symptoms. Additionally, it follows up closely on patients after a visit to a caregiver with daily questionnaires to ensure patients can recover at home without assistance.
We can divide the COVID-19 care pathway into 2 phases:
- Symptom check
- Tested positive for COVID-19
- Follow-up
The backbone of the care pathway is summarized in the figure below, for this flow we relied mainly on the ICHOM standard set of Patient-Centered Outcome Measures for COVID-19.
Symptom check
Based on the official guidelines, we check if the patient has COVID-19 symptoms. If the patients' symptoms do not match COVID-19, the care pathway automatically stops. If the symptoms do match, we ask that the patient be tested at the appropriate facility.If the patients' symptoms do not match COVID-19, the care pathway automatically stops. Baseline
If the patient tested positive for COVID-19, they get asked to fill in the following baseline patient-reported outcomes:
- Demographic Survey Questionnaire (e.g comorbidities, age, smoking status, etc)
- Patient-Reported Outcomes Measurement Information System (PROMIS-10)
- Influenza Patient-reported Outcome (FLU-PRO)
Follow-up
After baseline, the patient gets asked to fill in the following patient-reported outcomes at week 2, week 4, month 1, month 2 and month 3:
- Patient-Reported Outcomes Measurement Information System (PROMIS-10)
- Influenza Patient-reported Outcome (FLU-PRO)
