Overview of the care pathway
We can divide the gastric bypass care pathway into 3 phases:
- Baseline / intake
- First consultation
- Long-term follow-up post-treatment (until 1,5 years post-treatment)
Baseline / intake
The care pathway is automatically started when a consultation is booked with the patient. Two weeks before this consultation, the patient is asked to fill in the following questionnaires:
- Intake form (questions about medication, weight, height, if the patient is smoking, etc.)
- Bariatric Analysis and Reporting Outcome System (BAROS)
- Physical Activity Measurement (PAM)
Awell automatically calculates the BMI & PAM. We push those results together with the PROM/PREM responses to the EMR.
First consultation
In the first consultation, a healthcare professional can indicate what therapy needs to be followed:
- Conservative treatment
- Surgery
- Multidisciplinary program + surgery
Besides, they can also indicate to send a survey about their eating habits.
Follow-up
After treatment, we send follow-up PROMs/PREMs to the patients at 3 weeks, 4 months, 9 months, 1 year and 1,5 year follow-up. The following questionnaires are send:
- Bariatric Analysis and Reporting Outcome System (BAROS)
- Physical Activity Measurement (PAM)
- Follow-up questionnaire (questions about general health, quality of life, weight, height, etc.)
Awell automatically calculates the BMI & PAM. We push those results together with the PROM/PREM responses to the EMR.
