Overview of the care pathway
We can divide the pregnancy and childbirth care pathway into 5 phases:
- First prenatal visit
- Early 3rd trimester
- Prior to discharge from birth unit
- 1st postnatal checkup
- 6 months follow-up
The backbone of the care pathway is summarized in the figure below, for this flow we relied mainly on the ICHOM standard set of Patient-Centered Outcome Measures for pregnancy and childbirth.
