Chronic Pain Care Pathway

Overview of the care pathway

The chronic pain care pathway enables a multidisciplinary, integrated treatment of chronic pain patients. It ensures every caregiver (doctors, nurses, dieticians, psychologist, etc.) is up-to-date and gets involved when required.

Before each consultation, all relevant caregivers are notified after a holistic PROM assessment of the patient’s situation. When the team decides to opt for an intervention, the care pathway ensures a pre-intervention baseline and a 6 weeks post-op evaluation is collected.

We can divide the chronic pain care pathway into 2 phases:

  1. Baseline
  2. Treatment
  3. Follow-up

Baseline

The pathway is automatically started when an appointment is booked with the chronic pain center. At the start, the patient is asked to fill in the following patient-reported outcomes:

  • Demographic Survey Questionnaire (e.g comorbidities, medication use, smoking status, etc)
  • Pain questionnaire
  • Örebro Musculoskeletal Pain Screening (OMPQ)
  • 36-Item Short Form Health Survey (SF-36)
  • Social questionnaire (e.g financial status, home situation, professional situation, etc)

Treatment

After the first consultation, a healthcare professional can select one of the following treatment plans:

  • Interventional
  • Multidisciplinary program
  • Qutenza
  • Intravenous Therapy
  • Infiltrations and guided injections

Based on the type of therapy, the patient will receive specific information and patient-reported outcomes.

Follow-up

Three weeks after the end of treatment, the patient must complete a final evaluation. The healthcare professional can decide which patient-reported outcomes she wants to send to the patient:

  • Pain questionnaire
  • Örebro Musculoskeletal Pain Screening (OMPQ)
  • 36-Item Short Form Health Survey (SF-36)
  • Central Sensitization Inventory (CSI)
  • Tampa Scale of Kinesiophobia
  • Disability Rating Index (DRI)
  • Life Balance Inventory (LBI)
  • Multidimensional Pain Inventory (MPI)
  • Pain Coping Inventory (PCI)
  • Symptom Checklist-90-Revised (SCL-90-R)

After the patient completed the survey(s), the healthcare professional can decide to end the care pathway or start another treatment episode.
