Overview of the care pathway

We can divide the pregnancy and childbirth care pathway into 5 phases:

1. First prenatal visit

2. Early 3rd trimester

3. Prior to discharge from birth unit

4. 1st postnatal checkup

5. 6 months follow-up

The backbone of the care pathway is summarized in the figure below, for this flow we relied mainly on the ICHOM standard set of Patient-Centered Outcome Measures for pregnancy and childbirth.
