Situation


Enrollment in care management programs—like diabetes or chronic disease management—is a key step to ensure proactive care. But getting patients enrolled is a challenge. Coordinators receive a list of flagged patients and must manually reach out to enroll them.


Complication


The process is frustrating and time-consuming. Care coordinators spend hours manually calling patients, often leaving voicemail after voicemail with no response. Valuable time is wasted on follow-ups instead of meaningful patient care. Delays in enrollment mean delayed interventions, increasing the risk of patients’ conditions worsening or requiring hospital readmissions. The longer it takes to engage patients, the harder it becomes to keep them on track.



Resolution

If patients remain unreachable, the system flags the primary care provider (PCP) and creates a task for care coordinators to follow up.
